Race/ethnicity and challenges for optimal insulin therapy

AimsWe aimed to review insulin dosing recommendations, regulation and its determinants, glycaemic response carbohydrates, the efficacy safety of therapy in different races/ethnicities.MethodsWe searched for articles PubMed Google Scholar databases up 31 March 2021, with following keywords: “ethnicity”, “diabetes”, “insulin”, “history insulin”, “insulin therapy”, “food/rice”, “carbohydrate intake”, resistance”, “BMI”, dosing”, sensitivity”, response”, “glycaemic index”, “efficacy safety”, interposition Boolean operator “AND”. In addition, we reviewed reference lists found.ResultsThe differential effect race/ethnicity has not yet been considered current guidelines. Nevertheless, body size composition, mass index, fat distribution, diet, storage, energy expenditure vary significantly across populations. Further, sensitivity, response, carbohydrates differ by ethnicity. These disparities may lead requirements, adversely impacting among ethnic groups.ConclusionsRace/ethnicity affects glucose metabolism regulation. Until now, international guidelines addressing racial/ethnic-specific clinical recommendations are limited. Comprehensive updated ethnicity urgently needed.
