Race/ethnicity and challenges for optimal insulin therapy

چکیده
AimsWe aimed to review insulin dosing recommendations, regulation and its determinants, glycaemic response carbohydrates, the efficacy safety of therapy in different races/ethnicities.MethodsWe searched for articles PubMed Google Scholar databases up 31 March 2021, with following keywords: “ethnicity”, “diabetes”, “insulin”, “history insulin”, “insulin therapy”, “food/rice”, “carbohydrate intake”, resistance”, “BMI”, dosing”, sensitivity”, response”, “glycaemic index”, “efficacy safety”, interposition Boolean operator “AND”. In addition, we reviewed reference lists found.ResultsThe differential effect race/ethnicity has not yet been considered current guidelines. Nevertheless, body size composition, mass index, fat distribution, diet, storage, energy expenditure vary significantly across populations. Further, sensitivity, response, carbohydrates differ by ethnicity. These disparities may lead requirements, adversely impacting among ethnic groups.ConclusionsRace/ethnicity affects glucose metabolism regulation. Until now, international guidelines addressing racial/ethnic-specific clinical recommendations are limited. Comprehensive updated ethnicity urgently needed.

Despite advances in treatment for type 2 diabetes in recent decades, many patients are failing to achieve adequate glycemic control. Poor glycemic control has been shown to have a detrimental effect on patients' health and well-being, and to have significant negative financial implications for both patients and healthcare systems. Insulin therapy has been proven to significantly reduce glycated...
متن کاملIntensifying Insulin Therapy in Type 2 Diabetes: Choices & Challenges.
Insulin therapy remains the cornerstone of effective diabetes management. Timely intensification of insulin therapy reduces the progression of diabetes and the development of diabetes-related complications. Given that overall hyperglycaemia is a relative contribution of both fasting and postprandial hyperglycaemia, use of basal insulin alone may not achieve optimal glucose control due to its in...
متن کاملOptimal therapy for patients with hepatocellular carcinoma and resistance or intolerance to sorafenib: challenges and solutions
The only US Food and Drug Administration (FDA)-approved first-line systemic therapy for hepatocellular carcinoma (HCC) is sorafenib; however, resistance or intolerance to sorafenib is unfortunately common. In this review, we briefly describe systemic therapies that can be considered for patients with HCC who show resistance or intolerance to sorafenib. For all patients with HCC who need systemi...
